  Minimum Employment Requirements  
 
Current required licensure, board certification, etc. for clinical positions with at least 2 years recent experience in field.

Minimum score of 85% on knowledge assessment exam required for most clinical positions.

CPR Certification for Health Care Providers as required.

Advanced Practice Certification-Preferred (ACLS, CEN, CCRN, NALS, etc.).

Current PPD or CXR Report annually as required.

Current Annual Physical/Statement of Health as required.

Proof of Immunization or Immunity from Measles, Mumps, Rubeola & Rubella & Hepatitis B as required.

Completion of ongoing required annual training: Fire & Safety, HIPPA, Universal Precautions, OSHA, Age Specific Criteria, Right to Know, etc. as required.

CEU's, CME's, etc. as required.

2 above average letters of reference.

Social Security Card & Picture ID.

Initial and Random Ongoing Drug Screen and Criminal Background Check.

Some assignments may require eligibility for Federal Security Clearance.

Successful completion of facility specific training and orientation requirements as required prior to assignment.
